Introduction
Gestational diabetes mellitus (GDM) is glucose intolerance first recognized during pregnancy. Early diagnosis and appropriate treatment reduce maternal and fetal complications.
Blood Glucose Targets (Common Guideline Targets)
| Parameter | Target |
| Fasting | <95 mg/dL (<5.3 mmol/L) |
| 1-hour after meal | <140 mg/dL (<7.8 mmol/L) |
| 2-hour after meal | <120 mg/dL (<6.7 mmol/L) |
Figure (Placeholder): Treatment Algorithm
Diagnosis → Medical nutrition therapy + physical activity → Self-monitoring of blood glucose (1–2 weeks) → If targets not achieved → Pharmacologic therapy (usually insulin; oral agents may be considered in selected situations according to local guidance) → Ongoing maternal and fetal monitoring.
Guideline-Based Treatment Summary
| Guideline | Lifestyle | Preferred Medication | Metformin | Postpartum |
| ADA | First-line | Insulin if needed | May be used in selected patients after counselling | OGTT 4–12 weeks |
| ACOG | First-line | Insulin preferred | May be appropriate in selected patients | OGTT 4–12 weeks |
| NICE | First-line | Insulin when indicated | Accepted in selected patients | Postpartum glucose testing |
| FIGO | First-line | Insulin when indicated | May be considered in appropriate women | Long-term diabetes prevention |
Insulin Therapy
| Situation | Recommendation | Clinical Tip |
| Persistent fasting hyperglycaemia | Consider basal insulin | Adjust according to SMBG |
| Post-meal hyperglycaemia | Consider rapid-acting insulin | Match dose with meals |
| Multiple elevated values | Individualize regimen | Frequent follow-up |
Lifestyle Treatment
• Individualized medical nutrition therapy.
• Moderate physical activity if there are no obstetric contraindications.
• Self-monitor blood glucose as advised.
• Attend regular antenatal and diabetes follow-up.
Delivery and Postpartum Care
Delivery planning should be individualized based on maternal and fetal status. A 75-g oral glucose tolerance test is recommended 4–12 weeks postpartum to detect persistent diabetes or prediabetes, followed by lifelong periodic screening.
Key Messages
• Lifestyle therapy is the foundation of GDM management.
• Insulin remains the preferred medication when pharmacologic treatment is required in many guidelines.
• Blood glucose targets should be individualized.
• Postpartum follow-up is essential because women with GDM have an increased lifetime risk of type 2 diabetes.
