Steroid Withdrawal & Glucocorticoid Tapering (Expanded)

Long-term glucocorticoid therapy suppresses the hypothalamic–pituitary–adrenal (HPA) axis. Abrupt discontinuation may cause adrenal insufficiency or adrenal crisis. Tapering must be individualized.

Patients at RiskExamples
High riskPrednisolone ≥20 mg/day for >3–4 weeks
Moderate riskPrednisolone 5–20 mg/day for several weeks to months
Additional risk factorsRepeated courses, evening dosing, dexamethasone, Cushingoid features

Approximate Glucocorticoid Equivalence

DrugApproximate Equivalent Dose
Hydrocortisone20 mg
Prednisolone5 mg
Methylprednisolone4 mg
Dexamethasone0.75 mg

Illustrative Tapering Strategy

Current Prednisolone DoseExample Taper
>20 mg/dayReduce by 5–10 mg every 1–2 weeks if disease is controlled
20–10 mg/dayReduce by 2.5–5 mg every 2–4 weeks
10–5 mg/dayReduce by 1–2.5 mg every 2–4 weeks
<5 mg/dayReduce by 0.5–1 mg every 2–4 weeks or consider physiologic hydrocortisone if indicated

Important: This table is an example only. Tapering must be individualized according to the underlying disease, duration of therapy, relapse risk, and HPA-axis recovery.

Monitoring During Taper

AssessmentPurpose
SymptomsFatigue, weakness, nausea, dizziness, hypotension
Underlying diseaseDetect relapse
Morning serum cortisolAssess HPA-axis recovery in selected patients
Dynamic testingACTH stimulation test when clinically indicated

Sick-Day Rules

• Never stop replacement steroids abruptly.
• Increase glucocorticoid doses during significant illness according to your clinician’s instructions.
• Seek urgent medical care if vomiting prevents oral medication.
• Carry a steroid emergency card or medical alert identification.
